Recent scientific developments, in particular advances in pharmacogenetics and molecular genetics, have given rise to numerous predictive procedures for detecting predispositions to diseases in patients. This knowledge, however, does not necessarily promise benign results for either patients or health care professionals. The aim of this volume is to analyse issues related to prediction and prognosis as a burgeoning field of medicine, which is revolutionizing the way we understand and approach diagnosis and treatment. Combining epistemic and ethical reflection with medical expertise (...) on contemporary practice and research, an interdisciplinary group of international experts critically examine anticipatory medicine from various perspectives, including history of medicine, bioethics, theories of science, and health economics. The highly complex issues involved in medical prediction call for a far-reaching debate on the value and scope of foreknowledge. For example, which responsibilities and burdens arise when still healthy people learn of their predisposition to diseases? How should health care insurance reflect risky life styles? Is the increasing medicalization of life connected with prevention ethically sustainable and financially possible in the developing world? These and other related issues are the subject of this timely and important book, which not only serves as an introduction to the area, but also proposes many feasible solutions to the problems outlined. The most important modes of film practice, in my view, are art-house cinema and the avant-garde, both of which contrast with the classical Hollywood mode of film practice. While the latter is characterized by its commercial imperative, corporate hierarchies, and a high degree of specialization as well as a division of labor, the avant-garde is an “artisanal” or “personal” mode. Avant-garde films tend to be made by individuals or very small groups of collaborators, financed either by the filmmakers alone or (...) in combination with private patronage and grants from arts institutions. The aim of this paper is to highlight the decisive contribution of Simplicius and Philoponus to the resolution of the problem of evil in Neoplatonism. A correct and faithful interpretation of the problem, which also had to agree with Plato’s texts, became particularly needed after Plotinus had identified evil with matter, threatening, thus, the dualistic position, which was absent in Plato. The first rectification was made by Proclus with the notion of parhypostasis, i.e., “parasitic” or “collateral” existence, which de-hypostasized evil, (...) while at the same time challenging the Plotinian theory that turned evil into a principle that was ontologically opposed to good. In light of this, the last Neoplatonic exegetes, Simplicius and Philoponus, definitely clarified the “privative” role of kakon, finally relieving matter from the negative meaning given to it by Plotinus and restoring metaphysical monism. The paper presents my first findings and reflections on how ordinary people may opportunistically and unpredictably respond to Internet censorship and tracking. I try to capture this process with the concept of ‘hacking multitude'. Working on a case study of the Turkish government's block of the social media platform Twitter, I argue that during systemic data choke-points, a multitude of users might acquire a certain degree of reflexivity over ubiquitous software of advanced techno-capitalism. Resisting naïve parallels between urban streets and (...) virtual global streets, the article draws on Fuller's ‘media ecologies' to make sense of complex and dynamic interactions between processes and materialities, strategies of communication and mundane practices. Such a dense space is mostly invisible to network and traffic analysis, although it comes alive under the magnifying lens of digital ethnography. As the Turkish government tried to stop protesters on both the urban and the Twitter spheres, alternative material configurations and new hybrid formations and practices emerged. I try to bring this process alive following the traces – that is, a combination of digital data and materialities – of a social space between the protest for Twitter access, the ‘digital coup' and the interactions that this situation determined. In the final section, I briefly explore two research trajectories which can further develop my initial formulation of a ‘hacking multitude'. I argue that a generalisation of hacking/multitude is problematic for the political, cultural or economic processes more directly associated with Big Data. (shrink)

One of the main difficulties that Neoplatonic commentators of Aristotle face is the different treatment that the Categories and the Metaphysics offer to the question of the substance. After describing briefly the status quaestionis ousiae in Aristotle, and after tracing the main Neoplatonic interpretations of this doctrine, this article attempts to demonstrate that the Neoplatonists of Athens and Alexandria, Syrianus and Ammonius, inaugurate a new interpretation of the Aristotelian doctrine. With regard to the category of substance in general and to (...) the question of substantiality of “immanent form” in particular, this new interpretation goes beyond the positions of Plotinus and Porphyry and returns the ontological value to the Aristotelian substances. Unlike Plotinus, who recognized as ousia only that one intelligible, that is five genres of the Platonic Sophist, and unlike Porphyry, who defused the anti–Platonic fuse of the Categories, giving to this treaty a mainly semantic skopos, these philosophers, through their original study of the theory of the three states of katholou, already shed in the Porphyrian Eisagôgê, fit the immanent forms of Aristotle, recognized as substances and as a reflection of the transcendental universal, into the late antique Neoplatonic metaphysical triadic structure. Hospital acquired infections (HAIs) are a major threat to patient safety. This paper addresses the following question: given what is known about the causes of and possible interventions on HAIs, to whom or what should the moral responsibility for preventing these infections be attributed? First, we show how generating robust evidence on the effectiveness of preventive hygiene measures is a complex endeavour and review the existing evidence on the causes of HAIs. Second, we demonstrate that the existing literature on the (...) ethical aspects of infection control has focused on responsibility at the individual-level. Thirdly, we argue that these accounts do not accommodate systemic factors relevant for HAI prevention. We show that the notion of collective responsibility is useful for making understandable how systemic factors, such as employment conditions in hospitals, are both causally and ethically relevant in infection control. Background Female genital mutilation and circumcision (FGM/C) practices present physicians in Germany with numerous challenges. One possible intervention is elective clitoral reconstruction for esthetic and physiological recovery after FGM/C. Even if the study situation regarding the results achieved by clitoral reconstruction is controversial, the range of reconstruction options is increasing. Arguments The aim of this study is to critically examine the epistemic and ethical dimensions of the interdisciplinary debate on clitoral reconstruction that has arisen over the last 20 years. It (...) is intended to provide orientation in the complex field in which cultural, gender-specific, sexual, and psychosocial factors interact with anatomical–physiological knowledge and the lack thereof. The analysis shows the ambivalent role of evidence claims and promises in relation to clitoral reconstruction. Conclusions Difficulties in testing and comparing existing surgical procedures and in the use of standardized questioning tools demonstrate how essential cultural competence and diversity awareness are in dealing with those affected. The prima facie correct argument of insufficient evidence on the efficacy, comparability, and long-term consequences of reconstruction procedures proves to be problematic for ethical and epistemic reasons. In order to generate valid and useful data on sexuality, body perception, pain, and pleasure before and after clitoral reconstruction, survey modalities that address the culturally heterogeneous target group of women with FGM/C are needed. Cultural competence and sensitivity in medicine are also urgently needed for target group-oriented education and counseling as a prerequisite for self-determined decisions for/against clitoral reconstruction. Zusammenfassung Praktiken der weiblichen Genitalverstümmelung bzw. -beschneidung konfrontieren Ärzt*innen auch in Deutschland mit zahlreichen Herausforderungen. Eine mögliche Interventionsmöglichkeit ist die elektiven Klitorisrekonstruktion zur ästhetischen und physiologischen Rehabilitation nach weiblicher Genitalverstümmelung bzw. -beschneidung. Auch wenn die Studienlage bezüglich der durch Klitorisrekonstruktion erzielten Ergebnisse kontrovers diskutiert wird, steigt das Angebot an Rekonstruktionsoptionen an. Die vorliegende Arbeit zielt darauf ab, die epistemischen und ethischen Dimensionen der in den letzten 20 Jahren entstandenen und interdisziplinär geführten Debatte über die Klitorisrekonstruktion kritisch zu beleuchten. Sie soll (...) eine Orientierung in dem komplexen Feld bieten, in dem kulturelle, geschlechtsspezifische, sexuelle und psychosoziale Faktoren mit anatomisch-physiologischem (Un)Wissen interagieren. Die Analyse zeigt die ambivalente Rolle von Evidenz und Evidenzansprüchen in Bezug auf das chirurgische Verfahren der Klitorisrekonstruktion. Ethical issues are inherent in medicine. Morally appropriate forms of medical behaviour, the thorough communication of diagnosis and prognosis, and carefully evaluated treatment promising recovery have been among the standards of medical ethics down to the present day. The testimonies of a lively tradition, which since antiquity has contributed to the permanent critical reflection of medicine, constitute the cultural background of contemporary bioethics. They demonstrate how fertile the dialogue between medicine and philosophy on ethical questions can be. This also includes (...) the critical examination of the objects, aims, methods and boundaries of the physicians profession and of medical research. These boundaries have had throughout history to be constantly renegotiated and defined, and not only in the 21st century. --- Eine Reflexion uber die moralischen Fragen und Probleme, die die arztliche Praxis begleiten, ist der Medizin immanent. Die sittlich angemessene arztliche Haltung, die "gute" und hilfreiche Behandlung und das arztliche Ethos stehen im Zentrum zahlreicher Schriften, die zwischen Humanismus und fruher Neuzeit verfasst wurden. Es sind Zeugnisse einer lebhaften Tradition, die ununterbrochen seit der Antike zur Selbstvergewisserung der Medizin beigetragen hat. Zwischen Mittelalter und Humanismus sahen sich Mediziner den Herausforderungen des mannigfaltigen und lebhaften Heilkunde-Marktes ausgesetzt und dementsprechend verpflichtet, sich umso starker zu behaupten. Die hier prasentierten Beitrage befassen sich mit der uber sich selbst reflektierenden Medizin. Dazu gehoren die kritische Auseinandersetzung mit Gegenstand, Ziel, Methoden und Grenzen des arztlichen Berufes und der medizinischen Forschung. Moralvorstellungen generieren Erwartungen, die den Umgang mit den Mitmenschen prägen. Doch welches Verhalten ist kennzeichnend für einen integren, tugendhaften Menschen? In einer säkularisiert-pluralistischen Gesellschaft gelten Frauen und Männer als sozial kompetent, wenn sie sich durch Eigenschaften wie Ehrlichkeit, Zuverlässigkeit, Kollegialität und Vertrauenswürdigkeit auszeichnen. Diese Attribute haben die klassischen und christlich-theologischen Tugenden, etwa Tapferkeit, Keuschheit oder Mäßigung verdrängt. Die hier versammelten Beiträge fragen nach dem Einfluss expliziter und impliziter Tugendmodelle auf den Menschen, nach ihrer geschlechtlichen Prägung sowie nach den Geltungskonkurrenzen von (...) alten und neuen Idealvorstellungen. Aus der Perspektive des Subjekts betrachtet stellt sich die Frage, ob Tugendhaftigkeit wie ein Korsett je nachdem, von wem und für wen es geschnürt wurde stützend, formend, schützend, einengend oder sogar erstickend wirkt. *** Conceptions of morality generate expectations which inform our interactions with others. But what behaviour is typical of an integrated, virtuous person? In a secular and pluralist society men and women are deemed to be socially competent if they are distinguished by characteristics such as honesty, reliability, collegiality and trustworthiness. These attributes have replaced classical and Christian virtues such as courage, chastity and temperance. The essays collected here examine the influence of explicit and implicit models of virtue on people according to their gender imprinting and to the competing values of old and new moral conceptions. Viewed from the perspective of the subject, the question is raised as to whether virtuousness is like a corset and depending on the person for or by whom it is fastened can have a supporting, shaping, protective, constricting or suffocating effect. Das Wissen über das Virus und seine Eigenschaften ist lückenhaft, aber der Bedarf, politische und medizinische Entscheidungen an wissenschaftlicher Erkenntnis auszurichten ist groß. Diese Lage führt zu einer Dynamisierung der Forschung. Ein prominentes Beispiel ist die WHO-Studie SOLIDARITY. Die epistemologischen Besonderheiten und die daraus resultierenden ethischen Implikationen werden in diesem Beitrag näher beleuchtet.This paper is part of Forum (...) COVID-19: Perspectives in the Humanities and Social Sciences. The novel coronavirus poses a huge challenge to the world community. Knowledge about the virus and its properties is limited, but there is a great need to base political and medical decisions on scientific knowledge. This situation is leading to a dynamization of research. A prominent example of such a development is SOLIDARITY. The epistemological dimensions of this trial, which is coordinated by the WHO, and the resulting ethical implications are discussed in this article. (shrink)

Seeing the entwinement of social and epistemic challenges through COVID, we discuss the perils of simplistic appeals to ‘follow the science’. A hardened scientism risks excarbating social conflict and fueling conspiracy beliefs. Instead, we see an opportunity to devise more inclusive medical knowledge practices through endorsing experiential knowledge alongside traditional evidence types.

In times of ongoing resource shortages, appropriate evaluation criteria are crucial for the ethical prioritization of medical care. While the use of scoring models as tools for prioritization is widespread, they are barely discussed in the medical-ethical discourse in the context of the COVID-19 pandemic. During this time, the challenge of providing care for patients in need has promoted consequentialist reasoning. In this light, we advocate for the integration of time- and context-sensitive scoring (TCsS) models in prioritization policies that foster (...) treatment opportunities for patients with subacute and chronic conditions. We argue, first, that TCsSs enable a more efficient use of resources, reducing avoidable harm to patients by preventing arbitrary postponement of necessary but nonurgent interventions. Second, we contend that on an interrelational level, TCsSs render decision-making pathways more transparent, which promotes the information requirement of patient autonomy and raises confidence in the resulting prioritization decision. Third, we claim that TCsS contributes to distributive justice by reallocating available resources to the benefit of elective patients. We conclude that TCsSs promote anticipatory measures that extend the timeframe for responsible action into the future. This strengthens patients' ability to exercise their right to healthcare—primarily during times of crisis, but ultimately in the longer term too. (shrink)

The administrative reform of the datafied public administration places great emphasis on the classification, control, and prediction of citizen behavior and therefore has the potential to significantly impact citizen–state relations. There is a growing body of literature on data-oriented activism which aims to resist and counteract existing harmful data practices. However, little is known about the processes, policies, and political-economic structures that make datafication possible. There is a distinct research gap on situated and context-specific empirical research, which critically interrogates the (...) premises, interests, and agendas of data-driven public administration and how stakeholders can impact them. This paper therefore studies the conditions of participation in public administration datafication. It asks the overall research question of how citizens are problematized and included in policy and practitioner discourse in the datafication of public administration. The paper takes Norway as its case and applies Cardullo and Kitchin’s scaffold of smart citizen participation at the system level. It makes use of a unique empirical insight into the field, consisting of a survey, interviews, and an extensive document analysis. Unexpectedly, we find that citizens and civil society are rarely engaged in this administrative reform. Instead, we identify a paternalistic, top-down, technocratic approach where the context, values, and agendas of datafication are obscured from the citizen. (shrink)
